Diabetes is skyrocketing at an alarming rate across the United States. Not surprisingly, it's an even more significant health threat here in Louisiana where high-fat and fried foods seem ingrained in our culture.

The good news is that if you have diabetes or prediabetes, expert care is available close to home. Chalmette Medical Center's Diabetes Center, which is under the management of Jonathan Wise, M.D., an endocrinologist with the Diabetes and Metabolism Group, offers patients the comprehensive and personalized care they need to control this chronic disease.

The Center's team of diabetes specialists -- including endocrinologists, a dietitian and two certified diabetes educators (CDEs) -- assesses each patient and develops a care plan based on his or her individual needs. The team also refers patients to specialists at CMC as needed.

"We help patients manage their disease by controlling blood-sugar levels, high blood pressure, high cholesterol, obesity and other key factors," Dr. Wise says. "Our goal is to help prevent or delay the serious complications that are often associated with diabetes. People also feel better when their blood sugars are under control."

Education plays a vital role in the treatment process. The Center provides private educational counseling and a comprehensive, four-week diabetes program that is approved by the American Diabetes Association.

"We offer diabetics and their families information and support that will help guide them to make healthier choices and to live well with diabetes," says Debbie Jones, R.N., B.S.N., C.D.E., at the CMC Diabetes Center.
